NICE advice; relevant for rescue units, paramedics and medics?

All,
Have you seen the new NICE guidance on "Pre-hospital initiation of fluid replacement therapy in trauma"?
Especially the very first paragraph:
"IV fluid should not be administered if a radial pulse can be felt."

There has been a request to the MSA to look at reducing the amount of fluids a rescue unit has to carry (currently 6 litres), but have not heard what is happening.

I think the number of patients actually given IV fluids prior to Ambulance transportation - against actual number of patients transported anyway, is probably quite low anyway.

Cannulation is probably over-done by most Ambulance Trusts as well.

This is old news in trauma management but as always takes and age to filter down through the MSA. But certainly what is being taught on medical refreshers.

Most rescue units should now work to JRCALC guidelines up to their level of competence

That has been the advice for some time. See, for example, a 2002 paper http://emj.bmj.com/cgi/content/full/19/6/494 (will require free registration).
